Where your body stores and loses fat varies considerably between individuals, and breasts also contain glandular tissue. This means significant overall weight loss does not necessarily result in breasts becoming proportionately smaller.
A breast uplift primarily removes excess skin and reshapes existing breast tissue, whereas a breast reduction also removes breast volume to make the breasts smaller. If your breasts remain heavy as well as sagging after weight loss, reduction may be the more appropriate procedure.
